Driving license; application for a copy of a file card

The so-called index card transcript is an extract from the local driving license register of the driving license authority that issued your driving license and provides information about your personal driving license data.

It may be necessary, for example, if,

• you want to exchange your pink or gray paper driving license for the forgery-proof EU card driving license or
• Your paper driver's license has been lost and you need a replacement driver's license

and

• and your (last) driver's license was not issued by the driver licensing authority now responsible (= driver licensing authority responsible for your current place of residence), but by another driver licensing authority.

You can request a copy of the index card from the authority that issued your last driver's license.

This can usually be done by telephone, in writing or by e-mail. More and more authorities are also allowing online applications.

In order to be able to process the request as quickly as possible and without additional queries, the following information in particular is usually required:

• Family name
• Former surnames (maiden name or married name), if applicable
• First name(s)
• Date of birth
• Place of birth
• If known, driving license data (date of issue, driving license number)
• Address and, if applicable, fax number of the driving license authority now responsible for you.

As a rule, the copy of the record card is then sent directly to the driving license authority that is now responsible for you and where you applied for the new driving license.
